Intake Process

Before medical cannabis can be dispensed, follow these initial steps:

  1. Prescription
  2. Registration
  3. Verification
  4. Application

1. Prescription – Obtain a doctor’s written recommendation. Most doctor’s will prescribe medical marijuana for a wide-range of diagnosed physical ailments, internal diseases, and psychological issues. If your physician is not comfortable with this (or you’re not comfortable asking), feel free talk with us about direction.

2. Registration - Bring the following when visiting us for the first time:

  • Completed “ Membership Agreement and Code of Conduct.”
  • Your original (and current) doctor’s recommendation (yes, we’ll return it after our photocopy process).
  • You’re CA Drivers License or CA ID.*

*California State Medical Marijuana identification Cards are Not accepted.

NOTE: You MUST bring your Californa-Issued ID and your original recommendation with you on your first visit.

3. Verification – Wait for verification. We must verify your doctor’s recommendation before we can fill your first prescription. Depending on your physician, this part of the process may take several days.

4. Application – Complete your membership application and be in compliance with the California Regulations to receive medical cannabis. Upon verification, you will need to pay a one-time membership fee of $5.00. Then your membership and access to our dispensary services will become active. Medi-Cal patients and Retired Vets will receive a free membership. Your membership will be valid until the date your recommendation expires.

Additional Membership Qualifications  include:

**You must be at least eighteen (18) years old - Parents or court-appointed legal guardians must act as caregivers to obtain service on behalf of legally qualified patients who are younger than eighteen (18) years old.

NOTE: We serve only legally qualified patients and caregivers. Under no circumstances whatsoever are services provided to persons who are not qualified under California law. You must become a registered member to obtain services.

Designated Caregivers:

Hearts of Mt. Shasta Inc. patient members may designate a primary caregiver who is authorized to receive medicine on the patient’s behalf. Please be advised that a primary caregiver is someone who has “consistently provided for the housing, safety, or welfare” of the patient (CA H&S 11362.5) Under no circumstances may a primary caregiver who is authorized to receive medicine on the patient’s behalf. Please be advised that a primary caregiver is someone who has “consistently provided for the housing, safety, or welfare” of the patient(CA H&S Code 11362.5 Under no circumstances may a primary caregiver obtain services for him or herself unless he or she is also a legally qualified patient and registered as such with Hearts of Mt. Shasta Inc To designate a caregiver, a completed Primary Caregiver Designation form is required. Please ask one of the staff for this form. It will need to be filled out and notarized before we can allow you to access the dispensary services.

Any person who is not a member or qualified patient, including minors, is not permitted within the dispensary area of Hearts of Mt. Shasta.
